Behaviour Policy: 
CWMHA is committed to providing an environment in which all individuals are treated with respect. Members and participants of CWMHA shall conduct themselves at all times in a manner consistent with the values of the CWMHA which include fairness, integrity and mutual respect. Background Checks:

A new season is upon us and all volunteers working with children must have a police check completed and filed with the association.  As a reminder, OMHA policy requires all Team Officials, including Coaches, Assistant Coaches, Trainers and Managers to provide a Criminal Background Check as a condition of volunteering to work with children.While some consider this to be a technicality, CWMHA fully supports the OMHA’s Policy on Criminal Record Checks, as this important requirement helps to provide a safe and secure environment for CWMHA families and players. 
Vulnerable Sector Checks (Ontario Provincial Police) are required to be completed when a volunteer starts and every four years hence forth. Declarations are submitted annually. These forms are not available on-line, but are available by contacting CWMHA Risk Management Director, Kelly Sobol-Fraser.

nclude 2 pieces of Identification with 1 being a photo, along with your "Police Check" forms.
 
Examples: Birth Certificate, Passport, Hunting Licence, Driver's Licence.
  **No Health Cards.

Please contact Kelly Sobol-Fraser at [email protected] with questions.  Completed forms can be dropped to the CWMHA office off Pad A in Fergus.   We are not able to receive completed forms via email.  If you choose to take your forms directly to the OPP, you are responsible for any costs incurred.
