Travel Permit Request (Centre Wellington Minor Hockey)

I understand that this is a Travel Permit request for CWMHA Administration to submit to OMHA.
I understand that this is not an OMHA approval of a Travel Permit.
I understand that the team is not allowed to participate in the event until I have received an approved travel permit from OMHA.
- An approved travel permit will be sent via email to the email address provided above before the event.
I understand that CWMHA does not have the ability to approve travel permits.
I understand that CWMHA Administration cannot submit a travel permit until a roster has been submitted and approved, but this form can be filled out at any point in order to expedite the process of travel permit application
I understand that CWMHA Administration can be emailed at any time for clarification or follow up on the status of the travel permits
