Permission to Skate Forms and Non-Resident Passports 2019
Any member of CWMH at the Minor Atom age and up will need a Permission to Skate form to attend AAA 2019/2020 tryouts at one of our zone centres, Guelph or Waterloo.

Any member of CWMH at the Minor PeeWee age and up will need a Non-Resident Passport to attend one AA 2019/2020 tryout at the centre of their choosing.

This form is typically released by the OMHA on April 1st.  AAA tryouts may start April 15th. 

To obtain a Permission to Skate form for your child, please email Brandy at [email protected].  Any request received before April 3rd will be available on April 4th.  Any requests received between April 5-13 will be ready on April 14th.  It is your responsibility to ensure you understand the tryout procedure at either centre. 

Please ensure you include the following information:
1.  Player Name
2.  Date of Birth
2.  AAA Centre (Guelph, Waterloo or both)
3.  Previous Season Team

Anyone at the Minor PeeWee level and up who would like to try out for one AA centre will need a Non-Resident Passport.  Please email Brandy with the same info above.  Only one passport is able to be issued per player each season.
